Equipment & Furniture Relocation Form
To streamline the process for requesting office moves, IT has created a Microsoft Form. This form will be filled out by the requester and automatically routed to the appropriate folks for approval before being created as a ticket.
This article explains how the form works.
When do I need to fill out a form?
When you are:
Moving from one room to another.
Moving desks within the same room to another desk.
Moving your desk to a new location within the same room.
Moving your desk to a new location in a different room.
Basically, if any furniture or IT equipment is being moved, please fill out this form.

Note:
Classroom moves may have a higher priority near the beginning and end of the semester.
Even if approved, IT and Facilities will need to conduct a site survey of the area to determine if the relocation is possible.
The Form Process
When you are ready to begin a relocation request, go to this form and fill it out:

The form will walk you through questions such as what kind of move it is (classroom, office, etc.), where you are moving to, and more. Most importantly, it will ask you for your supervisor’s email address for the approval process.
When a form is submitted, it will automatically go through the approval process before being made into a ticket.
Approval Process
When you are required for an approval, two things will happen:
You will receive a Microsoft Teams notification requesting your approval.
Teams notification.You will receive an Outlook email requesting your approval.

You only need to approve one of these requests for the process to move forward:
Once approved, the form will move to the next person in the approval chain until it is completed. Once completed, a ticket will be created for IT to review the request and move forward if able.
